Payment Methods

Payment is requested in full at the time of service. Only credit major credit cards are accepted including those that are Health Savings Cards with credit card emblems on them. I will keep your card on file in your electronic medical record so it is safe and secure. I will submit your claim to CareFirst and Blue Cross Blue Shield if I am in-network with your plan. Monthly invoices can be provided for insurance purposes if requested.  

I am in-network with most CareFirst PPO and BCBS PPO plans.  It is your responsibility to contact your plan to see if I am in-network.  Please contact your plan and check to see if Joseph LaFleur is a provider.  If I am an in-network provider, request information on co-pays and possible in-network deductibles.

All associates services will be billed under Joseph LaFleur, MSW, LICSW

I am considered Out-of-network for most plans.  PPO and Open Access Insurance Plans cover some of the cost of therapy services. How much they cover will depend on your plan and deductible.  Patients are responsible for checking with their insurance company for benefit coverage. Every insurance provides different coverage and reimbursement rates.  The most likely CPT codes used are 90791 and 90837; your insurance will request this information when you contact them for benefits. I will provide you with an invoice with all the necessary information for you to file with your insurance.

Although I am considered out-of-network with your health plan, I am happy to submit it electronically to your insurance company on your behalf, if I can. It will save you the paperwork.  Once I file electronically your insurance will reimburse you. If you prefer, I can also provide you with an invoice with all the necessary information for you to file with your insurance.  We can discuss this in greater detail during your appointment.
